MISSION:

To promote independence and reside in the least restrictive living environment.

The Family Assistance & Support program for children saw the program increase by 5 clients or 100%.

100% of the individuals in this program maintained their living environment.

FAS-Adult program had a total of 23 goals set with 19 being recorded as met.

FAS-Child program had a total of 35 objectives set with 20 being met.

HBSS had a total of 61 objectives with 38 being met at a rate of expectancy range.

COMMENTS RECEIVED:

Want more staff time; staff are very resourceful; 100% satisfied; does not like staff offices being cramped/no private place to talk.

STRENGTHS:

  • Individuals receive one-on-one support with staff who have worked with them for a lengthy time period and know them well.
  • In-home services are greatly needed and appreciated by individuals served. When one staff is on vacation, another is available to assist and the individuals served do not miss any of their services during vacation leave.
  • Services allow individuals to continue to live in the community in a home-based setting vs a more restrictive environment.

WEAKNESSES:

  • The agency does not have the authority to choose in-home support workers.
  • The children’s program has a very high staff turnover rate.
  • In home workers (HBSS) are not paid/reimbursed in a timely manner – memo was sent to all workers explaining the process.
